A teenage rapist at Wetherby Young Offender Institution sparked a security alert after using a spoon to tunnel to a neighbouring room to attack another criminal.
The convicted rapist is thought to have used the sharpened spoon to break into the cell next door and attack a convicted murderer, leaving him with serious injuries.
Bosses at the institution in West Yorkshire are now investigating the incident in the segregation unit, according to The Sun.
A source told the newspaper: ‘This is a major security breach. What is really worrying is that he simply got through a wall to launch the attack.
‘An inquiry has been launched to find out how the wall could be broken through and why nobody reacted in time.’
Governors are reportedly questioning guards about why the ‘noisy’ disturbance went unnoticed by security.

The Youth Custody Service said: ‘While we cannot comment on a live investigation, anyone found responsible for violence will be punished.’
It comes one month after a report found a girl at the institution had her clothes removed by adult male officers.
The prisons watchdog said he was ‘deeply shocked’ after the vulnerable teenage girl was forcibly strip-searched at the child jail.
Carlie Taylor, HM Chief Inspector of Prisons, also blasted ‘inappropriate use of pain-inducing techniques’ at the institution.
The institution was holding 165 boys and girls aged 15 to 18, including some of the country’s most ‘challenging’ young offenders, when inspectors visited at the end of last year.
